How Nourish Transparency works

On the Nourish platform, needs assessment and care delivery are connected through the same care and support records. We call these building blocks interactions, the individual notes, tasks and observations logged as care happens. The activities used to describe a person’s needs are also used to plan and record the care provided, creating a clear line of sight from assessed need to delivered care. This enables organisations to continuously review whether support remains aligned with reality as circumstances change.

Nourish Transparency makes change visible without imposing surveillance or unsustainable administrative burden. Through approaches such as dynamic sampling, it surfaces trend, drift and escalation early enough to support timely, proportionate action.

Dynamic Sampling continuously monitors care delivery to identify where meaningful change is occurring. Rather than repeatedly reassessing every person, it intelligently focuses attention where care demand is evolving, providing earlier insight while minimising administrative burden.
