The Feebris integration can help care teams monitor a wide range of conditions. Ranging from frailty to long-term conditions such as heart failure, COPD and diabetes. Across all of them, recognising deterioration early can change the outcome entirely. The signs are often subtle. Small shifts that mean little on their own but, tracked over time, become a clear signal that something needs attention.
